This is for the Dinner portion of the meeting and the Continuing Education Classes. Please select this option if you WILL be staying for the Continuing Education Classes. Please fill in ALL of your license numbers so that we may prepare your certificate of completion.

This is for the Dinner portion of the meeting only. Please select this option if you will NOT be staying for the Continuing Education Classes.
